Rachael holds Buddy's head up because he is very
sleepy and his head needs to be high enough for
Doctor Nigel to look in his mouth.

Doctor Nigel gets his big rasp out and starts to file
all the rough bits off Buddy's teeth.
If his teeth are left rough, Buddy would not be
able to chew his food properly and his mouth
would get very sore.
