Deadline is Monday at 5 p.m. The South East Local Health Integration Net- work (LHIN) is inviting feedback on Patients First: A Proposal to Strengthen Patient Centred Health- care, which was released on December 17, 2015 by the Ministry of Health and Long Term Care (MOHLTC). Patients First is a dis- cussion paper that outlines proposed changes for the health system. The pro- posed structural changes would see LHINs assume responsibility for home care and the planning and monitoring of primary care, and have improved linkages with population health planning. One new proposal is to make it easier for patients to find a primary health care provider when they need one, see that person quickly when they are sick, and find the care they need, closer to home. Opportunities are being provided by the South East LHIN for members of the public to learn more about the proposal, as well as to provide feedback. An online survey is available at www.south- eastlhin.on.ca, between February 1 - 26, 2016, and a phone-in town hall will be offered on February 19, 2016. Find more details at www.southeastlhin.on.ca. A series of meetings to learn more and provide feedback are Feb. 9 in Brockville, Feb. 10 in Belleville; Feb. 16 in Kingston, Feb. 22 in Perth and Feb. 23 in Kingston (French). Contact the LHIN at 1- 866 831-5446 for more or to register. LHIN looking for feedback on health care reforms Health reforms should facilitate smoother transitions: Crapper •Continued from page 1 were served at their Westport location. The new Westport Country Roads team are still in the transition of having their first appointments with the site's clients. "Our plan is to continue to focus on the folks who came from CPHC," said Crapper. "The medical providers have to review those files. If you have someone with a whole complex medical histo- ry it does take a lot longer to process new folks," he said. Crapper sees the newly orphaned patients as an example of one of the reasons the organization of health care services is currently being re-organized by the Min- istry of Health. "We want to see that never do patients have to look for a health care provider. We want to make sure that when a doctor retires they are part of a team and that team has the responsibility for ensuring there is a replacement," he said.
