LEGAL: IMPORTANT INFORMATION It is beneficial to have all important documents secured in a fire safe location. ITEM RESPONSE POWER OF ATTORNEY Legal Firm Name of Lawyer Phone Number Responsibilities YOU SHOULD KNOW… A power of attorney is a signed document that gives one person the authority to make decisions for someone on behalf of that person. This document provides the security that you may need to make decisions for your family, whether it is financial, medical, educational or legal. You should also keep a photocopy of the document, along with the original, in a safe place in your home. You should consider what your power of attorney covers once you have it completed. A power of attorney completed with the military will not cover all civilian matters, and vice versa. A good idea would be to ask questions, and apply for both. Continuing Power of Attorney for Property: gives someone else the legal authority to make decisions about your financial affairs/property, and allows that person to act on your behalf if you become mentally incapable. Non-Continuing Power of Attorney for Property: gives someone else the legal authority to make decisions about your financial affairs/property, in certain situations and for a specific period of time; it is automatically revoked if you become mentally ill. Power of Attorney for Personal Care: gives someone else the legal authority to make personal care decisions on your behalf if you become mentally incapable (e.g. related to housing and health care; banking) it is recommended that you contact your bank(s) to discuss if they require a specific banking power of attorney that needs to be completed. Each bank has different policies.
