A hearing by the Col- lege of Physicians and Surgeons Monday to decide on discipline for Dr. Ian Shiozaki has resulted in the suspension of his registration status effective March 12. A six-month suspension has been imposed which is to be followed by a six- month period when the doctor may practice only under the supervision of a college approved clinical supervisor, according to the College. Shiozaki is to abide by all recommendations of his clinical supervisor with respect to his practice, including but not limited to practice improvements, practice management, and continuing education. The period of Clinical Supervi- sion will begin on the expiry of the period of sus- pension, or on the date that the clinical supervisor is approved. He will not be allowed to subscribe a number of pain related medications and is restricted in the sub- stances he is allowed to inject. The college's hearing found Shiozaki guilty of the allegations that he engaged in professional misconduct and/or incom- petent in his family prac- tice, and engaged in dis- graceful, dishonorable or unprofessional conduct by failing to comply with the College's Out of Hospital Premises requirements. The decision is a result of an investigation begun in October when the Col- lege was contacted by a physician with informa- tion regarding the dosages of a stimulant prescribed by Dr. Shiozaki to a mutu- al elderly patient. That and other informa- tion prompted the college to begin an investigation. A review of 25 patient charts by a family medi- cine specialist retained by the college, as well as two interviews with Dr. Shiozaki by the specialist, identified a number of concerns, according to the College. They included issues around the prescribing of controlled drugs, includ- Seeley's Bay property owners have been receiv- ing notices of the need to have their septic systems inspected as part of the township's Septic Rein- spection Program. The township has com- pleted waterfront property inspections throughout the township and is turning the program's attention to hamlets where with small lots there can be health risks with the close prox- imity of wells and septic systems. The program is admin- istered by the Lanark Leeds and Grenville Pub- lic Health unit since 2009 and requires, at the owners cost to have an inspection completed by either, a cer- tified sewage system con- tractor, certified consul- tant or an inspector with the Health Unit. A report is to be com- pleted on the approved form and forwarded to the Health Unit by Aug. 15 of that year. Non-compliance comes with a cost. If a report is not received by August 15 the Health Unit shall issue a letter advising that an inspection will be con- ducted by a certified inspector and that costs plus an administration fee of 15 per cent will be charged back to the indi- vidual. In 2017 the by-law was changed to allow a shorter timeline for compliance to increase the number of properties which would be inspected each year.
