Providers can quickly review or access: All measures Measures with performance not met with action required Successful measures with performance met Number of Care Gaps Open at the Patients level and at the Care Gap level Patient Roster with New, Lost, and At Risk Patients Key Metrics Quality Measures 18.3 helps providers track several key metrics related to patient outcomes: Quality Measure Performance: Measure Met: A subset of the Total Number of Quality Care Measures where the performance percentage is equal to or greater than the designated performance percentage threshold. Measure Not Met: A subset of the Total Number of Quality Care Measures where the performance percentage is less than the designated performance percentage threshold. Care Gaps Status: Patients with Care Gaps: Total number of patients that have at least one care gap open over the total number of patients that are eligible for care gaps (patients who have either closed or open care gaps). Care Gaps Open: Total number of care gaps across all eligible patients that have not been closed and the total number of care gaps across all eligible patients that are either open or closed.
