Commercial Module: Summary | Elements of the Commercial Summary Page June 2018 Quality Measures 18.3 2-4 User Guide v1.4 Circle Graphs The circle graphs on the Summary page display open care gaps at the patient level and at the care gap or measure level. For example, one patient may have one or more care gaps open. Patients with Open Care Gaps This circle graph displays the number of patients with care gaps open, as determined from payer gap reports. Within the graph, the top number is the number of patients with open care gaps, which are measures that have not been met. The bottom number is the total number of patients who have measures that have been met and measures that have not been met, or in other words are eligible for a care gap. Click the numbers to view those patients with care gaps not met. Open Care Gaps This circle graph represents the different care gaps. The Patients with Care Gaps Open circle graph rolls up to this one. The top number is the number of care gaps that are open, and the bottom number is the total number of care gaps, open or closed.
