This programme offers support and advice to you and your family if you have
been hospitalised with a cardiac condition, event or surgery. Support and advice
are a phone call away.
You are always encouraged to follow the advice given to you by your doctor.
However, there are times when you may be unsure about how you feel or the
symptoms you may experience following a cardiac admission.
This is the time to make use of trained professionals for support and / or advice.
PROGRAMME BENEFITS
• Telephonic support from a Cardiac & Cardiothoracic trained ICU sister
• Education on diet and lifestyle changes to enable you to maximise your health, going
forward
• Advice and assistance in registering your condition for Chronic medication,
consultations etc.
• Ongoing support and counselling for you and your family
• Members who were admitted with a cardiovascular disease such as hypertension,
hyperlipidaemia or stroke are referred post discharge from hospital to the
Hypertension/Hyperlipidaemia case managers for follow up and registration on their
programme.
The programme co-ordinator automatically contacts you if you have been hospitalised
with a cardiac condition, event or surgery. However, members as well as their family
members are welcome to call during office hours (08h00–16h00) for support and advice
on 0860 100 544.
